In IUI, specially prepared, high-quality sperm are placed directly into the uterus at the time of ovulation, bringing them closer to the fallopian tubes and shortening the distance they need to travel to fertilise the egg. Unlike IVF, IUI does not involve egg collection or embryo transfer; fertilisation occurs naturally inside the body, making it a less invasive and more affordable option for many couples.
IUI can be performed during a natural ovulation cycle or with the help of ovulation stimulation medications such as Clomiphene Citrate or Letrozole. When medications are used, the cycle is closely monitored with ultrasound scans and blood tests. Once the dominant follicle is mature, a trigger injection is given to release the egg, and the IUI procedure is scheduled 24–36 hours later. Sperm preparation involves the male partner producing a sample in the clinic about two hours before the procedure. The sample is washed in the laboratory to select the healthiest, most motile sperm, which are then inserted into the uterus using a thin catheter. The procedure is usually painless, similar to a smear test, although mild cramping or discomfort can occur. Patients may rest afterwards if they wish.
IUI can be an effective option for couples with unexplained infertility, mild male factor infertility, or difficulties with sexual intercourse. It is not generally recommended for cases of severe male factor infertility, blocked fallopian tubes, advanced endometriosis, or significant pelvic adhesions.
Success rates per IUI cycle are typically lower than IVF, averaging 8–15% per cycle, 18% cumulatively after three cycles, and up to 30% after six cycles. Fertility medications can improve success rates but also increase the chance of multiple pregnancies, which carry additional risks such as premature birth and pregnancy complications. Guidelines from ESHRE and ASRM suggest limiting IUI to no more than 3–6 cycles if pregnancy is not achieved. Studies also show that for couples with unexplained infertility, stimulated IUI is more effective than natural cycle IUI or expectant management.
IUI is minimally invasive, lower in cost, and may require multiple cycles to achieve pregnancy. IVF, by comparison, has higher per-cycle success rates but involves more invasive procedures, higher costs, and controlled embryo transfer to reduce the risk of multiples.
