IVF and other assisted reproductive technologies (ART) have revolutionised fertility care but have also increased the risk of multiple pregnancies. While twins or triplets may be a joyful outcome, it’s important to understand the significant health risks involved for both the mother and the babies. At Almond Blossoms, we are committed to promoting healthy pregnancies and reducing avoidable complications by adhering to international best practices, including elective Single Embryo Transfer (eSET) whenever medically appropriate.
Naturally, twins occur in about 1 in 80 pregnancies. In contrast, with fertility treatment, this can increase to 1 in 5 IVF pregnancies worldwide, depending on how many embryos are transferred. Multiple pregnancies can occur in two main ways:
Non-identical (dizygotic) twins: Two embryos implant at the same time. This is the most common IVF outcome.
Identical (monozygotic) twins: A single embryo splits after transfer. This can happen even with a single embryo transfer but is less common.
While some twin pregnancies progress smoothly, research shows that multiple gestations carry significantly higher risks:
For the mother:
First trimester miscarriage
Iron deficiency and general fatigue
High blood pressure (preeclampsia)
Bleeding during and after pregnancy
Caesarean delivery (required in over 97% of twin pregnancies)
For the babies:
Premature birth (preterm delivery)
Low birth weight
Higher risk of neonatal intensive care admission
Bowel complications (e.g., necrotizing enterocolitis)
Long-term issues like cerebral palsy, learning difficulties, and delayed development
At Almond Blossoms, we prioritise the health of both mother and baby. Following ESHRE and ASRM guidelines, we implement a policy of elective Single Embryo Transfer (eSET) wherever possible. We especially recommend transferring a single embryo in cases where embryos have undergone genetic testing and are known to be chromosomally normal, as these embryos already have a high potential for implantation and success. In specific cases, after a careful clinical and embryological review, and discussion with the couple, transferring more than one embryo may be considered. Please note: As per Federal Law in the UAE, a maximum of two embryos may be transferred per treatment cycle, regardless of patient age.
