Understanding the Natural Cycle and Ovarian Stimulation

In a typical menstrual cycle:
The pituitary gland releases Follicle-Stimulating Hormone (FSH), prompting several ovarian follicles (fluid-filled sacs containing immature eggs) to grow. Usually, one follicle becomes dominant, maturing fully while others stop developing. As the dominant follicle grows, it produces estradiol (E2), which helps prepare the uterine lining for potential implantation. A surge in Luteinizing Hormone (LH) then triggers ovulation, releasing the mature egg into the fallopian tube.

In In Vitro Fertilization (IVF), we aim to retrieve multiple mature eggs to increase the chances of successful fertilization. To achieve this, hormonal medications (FSH and sometimes LH) are administered to stimulate the ovaries to mature multiple follicles simultaneously. This process is carefully monitored through ultrasound scans and blood tests to assess follicle growth and hormone levels. At Almond Blossoms, we tailor stimulation protocols based on age, Body Mass Index (BMI), hormone levels, ovarian reserve, and previous treatment history. This personalized approach aligns with the European Society of Human Reproduction and Embryology (ESHRE) guidelines, emphasizing individualized care to optimize outcomes and minimize risks.

Trigger Injection, Egg Collection, and Safety

Once follicles reach the appropriate size (typically around 18mm), a trigger injection is administered to finalize the maturation of the eggs and schedule egg retrieval approximately 36 hours later, ensuring eggs are collected before natural ovulation occurs. The egg collection procedure is performed under sedation or general anesthesia, using a transvaginal ultrasound-guided needle to aspirate fluid from each mature follicle. An embryologist examines the fluid to identify and collect the eggs. The number of eggs retrieved can vary and may be more, fewer, or exactly as expected based on monitoring.

Ovarian stimulation is generally safe and well-tolerated, especially when carefully monitored. However, potential risks include Ovarian Hyperstimulation Syndrome (OHSS), poor ovarian response or cycle cancellation, and unexpected ovulation.
