What Is IUI?

Intrauterine insemination (IUI) is a type of fertility treatment where specially prepared, high-quality sperm are placed directly into the uterus at the time of ovulation. This brings the sperm closer to the fallopian tubes, shortening the distance they need to travel to fertilise the egg.

Unlike IVF, IUI does not involve egg collection or embryo transfer. Fertilisation occurs naturally inside the body, making IUI a less invasive and more affordable option for many patients.

How Does IUI Work?

IUI can be done either:

  • During a natural ovulation cycle.
  • With the help of ovulation stimulation medications (e.g., Gonadotrophins, Clomiphene Citrate or Letrozole).

When medications are used, your cycle is closely monitored with ultrasound scans and blood tests. Once the dominant follicle is mature, a trigger injection is given to release the egg, and the IUI procedure is scheduled 24–36 hours later.

Sperm preparation: Your husband is scheduled to produce a sample in the clinic 2 hours before the procedure. The semen sample is washed in the laboratory to select only the healthiest, most motile sperm. This concentrated sperm is inserted into the uterus using a thin catheter.

The procedure is usually painless, similar to a smear test, though some women may care mild cramping or discomfort. Patients are welcome to rest afterwards if needed.

Who Should Consider IUI?

IUI may be a good option for:

  • Couples with unexplained infertility.
  • Couples with mild male factor infertility (eg. borderline sperm count or motility).
  • People with sexual dysfunction or difficulty with vaginal intercourse (eg erectile dysfunction or vaginismus).

IUI may not be suitable if:

  • There is severe male factor infertility (very low sperm count or poor quality in general).
  • The woman has blocked fallopian tubes.
  • There is advanced endometriosis or significant pelvic adhesions.

What Does the Research Say?

  • Success rates per cycle are generally lower than IVF.
  • Around 8–15% per cycle (it can rise to 30 percent per cycle for couples under 35).
  • 18% cumulative pregnancy rate after 3 cycles.
  • Up to 30% after 6 cycles.
  • Fertility medications can increase success rates, but they also carry a risk of multiple pregnancies, especially if more than one follicle matures. This increases the chance of twins or more, which comes with added risks like premature birth and pregnancy complications. Please refer to the multiple pregnancy leaflet for more information.
  • According to ESHRE and ASRM, IUI can be considered as a first-line treatment in suitable patients, but should not be extended beyond 3–6 cycles if pregnancy is not achieved.
  • Studies also show that for couples with unexplained infertility, stimulated IUI is more effective than natural cycle IUI or expectant management.
